Logistics Volunteers and Science Activity Volunteers must both complete the following:

  1. Read your volunteer position description.
  2. Fill-out the Volunteer Designation Form and submit it through campus or regular mail to the address below on or before 5 pm Thursday, February 27, 2020. After that date, bring your form to the Science Carnival. Please review the Sample Volunteer Designation Form for additional guidance in completing the form. Please make sure to include the following on the Volunteer Designation Form:
    • The specific work location is Rio Vista Middle School
    • Driving to and from the Science Carnival is not considered driving a vehicle on University business
    • Driving to and from the Science Carnival is not considered traveling on University business
    • The signature of volunteer must be a real signature
    • Please feel free to answer "yes" or "no" if you are receiving academic credit for volunteering and if you are a University staff, student or faculty member, per your specific situation
  3. Complete the survey to certify that you have completed the above training and that you understand the risks associated with your volunteer role at the Science Carnival by completing the Training Certification Survey (Survey is closed as of Thursday, February 27, 2020; you will have an opportunity to complete the survey as a hard copy version on-site). This link will be active through 5 pm Thursday, February 27, 2020. After that date, you will complete a paper version of the survey on site at the Science Carnival. Make sure that you view the video, however, prior to arriving at the Science Carnival.
All individuals must provide the Volunteer Designation Form to be able to participate as a volunteer at the Science Carnival.
Students who are minors must be supervised by an adult and they must provide the Volunteer Designation Form with parent signatures and parent emergency contact information.

Forms can be sent through CI campus mail to Lori Knudson, El Dorado Hall, brought to the Science Carnival on the day of the event, or sent by regular mail to the below address:

Copies of these forms will be available at the Science Carnival; however, you must view the training video by 5 p.m. Thursday, February 27, 2020.

In addition to the above tasks, Science Activity Volunteers must complete the following:

Activity Volunteers

  1. Volunteers must receive training from your Zone Leader who will be in contact with you in early to mid-October. You must also complete all items listed under the Activity Volunteers section. Training must be completed by Friday, October 25, 2019.
  2. Read and understand the background information about your assigned science activity at the on-line Science Carnival Activity website. Be sure to read any materials related to your activity well in advance of the Science Carnival and send any questions to Prof. Hampton or to your Zone Leader.
  3. Review the hazards associated with your science activity and read the MSDS forms for chemicals that will be used in your science activity.
  4. View a shortened version of the CI Chemistry Department Safety Video.
  5. As part of the Training Certification Survey, you will be asked whether you viewed these materials and whether you understand the hazards associated with your activity.
  6. Certain activities may require additional training, especially those involving cryogens, chemicals, or fire. This training must be provided by Prof. Hampton or the Chemistry Laboratory Technicians.
