Five Step Student Organization Renewal

Student Organizations that wish to renew for the 2012-2013 academic year must complete the renewal process listed below.  

 

Step 1 - Student Organization Renewal Application

Submit your Student Organization Renewal Application (SORA) located at https://cidsa.wufoo.com/forms/student-organization-renewal-form-2012/.

Please note:  In order to complete this form, you will need:

  • Officer, member and advisor names, student ID numbers and MyCI email addresses (no other email addresses accepted).
    • It is required that all organizations have a President and Treasurer on record with Student Leadership Programs in order to be registered.
  • A description of the purpose of the student organization
  • The most recent copy of your constitution (to be uploaded on the SORA) 

Additionally, on page two of this form, you will be asked to submit information about your student organization's activities during the 2011-2012 academic year. You will need the following information to complete the End of Year Report

  • Number of members, officers, events and meetings during 2011-2012
  • Accomplishments and challenges experienced during 2011-2012
  • Service projects completed during 2011-2012 including the number of members who took part in service and the number of hours they served (this is used for the President's Honor Roll and is required for completion of this form)
  • The goals your group has for the 2012-2013 year

Step 2 - President Agreement

Student Organization Presidents must submit a President Agreement form located at: https://cidsa.wufoo.com/forms/president-agreement-20122013/,

 

 Step 3 - Advisor Agreement

Student organization advisors must submit a Advisor Agreement form located at:https://cidsa.wufoo.com/forms/advisor-agreement-201213/. 


Step 4 - Officer Agreement

All student organization officers (students who are elected or appointed to a position) must submit the officer agreement form located at https://cidsa.wufoo.com/forms/officer-agreement-form-oaf/.

In order to verify this form was completed by the individual who it refers to, the student will be sent a confirmation message to his/her MyCI email address when the form is received. The student is required to reply to the confirmation email to verify that he/she completed the form.

Minimum eligibility requirements to be an officer of a student organization at CSU Channel Islands are as follows:

  • Must be enrolled in no less than six units at CSU Channel Islands (undergraduates)
  • Must be enrolled in no less than three units at CSU Channel Islands (graduate and credential)
  • Have a cumulative, CI and semester GPA of at least 2.5 (undergraduates)
  • Have a cumulative, CI and semester GPA of at least 3.0 (graduate and credential)
  • Be in good academic and behavioral standing (not on academic or disciplinary probation)
  • Be free of any holds on University records.

Step 5 - Officer and Advisor Orientations

All Student Organization Officers must submit the Student Organization Officer Orientation Evaluation (which will be available beginning May 20, 2012) by the third Friday of the Fall semester or no later than 10 business days after the officer assumes his/her position (in the event of an officer change).

Student Organization Advisors must submit the Advisor Orientation (which will be available beginning May 20, 2012) by the third Friday of the Fall semester or no later than 10 business days after the advisor assumes his/her position (in the event of an advisor change).

Sports Clubs

All sports club officers and members must submit the following Sports Club Participation Application https://cidsa.wufoo.com/forms/player-participant-application-2012/ and print, sign and submit the Release of Liability and Health Declaration (PDF, 79KB) forms to the SEAL Center located on the second floor of the Student Union.

  • In order to verify this form was completed by the individual who it refers to, the student will be sent a confirmation message to his/her MyCI email address when the form is received. The student is required to reply to the confirmation email to verify that he/she completed the form.
  • Please note that students must have medical insurance in order to participate in a sports club.

 

Renewal Timeline

Student organization renewals are due the first Friday after the Spring semester ends (May 31, 2012).  The status of organizations with late applications will be determined on a case-by-case basis.  

The following must be received by the deadline stated above in order to recieve the benefits of a registered student organization (participation in Island View Orientation, participation in the officer appeals process, and access to funding and other resources):

  • Student Organization Renewal Application (SORA)
    • A minimum of 5 students who will serve as officers and/or members
    • A president and treasurer
    • An up-to-date constitution
    • Details pertaining to service projects that took place during 2011-2012
    • An advisor
  • Officer Agreement Form
  • President Agreement Form
  • Advisor Agreement Form
  • Player Participation Form (for sports clubs)

The following must be submitted by the third Friday of the Fall semester to retain registered status:

  • Officer orientation evaluation completed by all officers including the president
  • Advisor orientation evaluation
