Various forms and documents are available to assist with the logistics involved with managing your student organization.

|Add/Drop Process||Student Leadership Programs must be notified when officers or advisors are added or removed to a Student Organization. Please follow this link to review the Add/Drop Process.|
|Advisor Agreement||This form should be submitted by the Friday after the Spring semester ends or no later than 10 days after assuming advisor role (in the event of advisor changes). When your advisor's name and email have been updated in your student organization's CISync portal, the new advisor will be sent the Advisor Agreement. Please follow this link to review the process for adding an advisor to your organization|
|Budget Report Request Form||This form is for requesting a budget report of the current financial status of a Student Organization that has money from fundraising in an ASI account. This form is located in CI Sync (see instructions above).|
|Budget Forms||Since student organizations (with the exception of sports clubs) are funded by Associated Students Incorporated, the ASI website contains budget-related information. Follow this link to access other ASI budget-related forms.|
|Chalking Request||This form is provided for student organization that wish to advertise through the use of chalking on the ground (in pre-approved locations). This form is located in CI Sync (see instructions above).|
|Event Space & Resource Request Form||
This form must be submitted for any event when the Student Organization is requesting the use of resources beyond space, if more than 100 attendees are expected at the event, if the event is publicized to off campus attendees or if food will be served. To request space for a meeting, complete the Meeting Space Request form. To request a table for tabling, complete the Tabling Request Form. You will receive an email confirming your space reservation. This form is located in CI Sync (see instructions above).
|Exploration of Interest Form||
If student organizations do not have the items necessary to start a registered student organization but do have five interested students, the group can apply for Exploration of Interest Status. Exploration of Interest Status allows the group to secure meeting space and advertise their group on campus. This will facilitate the process for them to recruit members, an advisor and receive assistance/additional time with completing the registered process.
|Fundraising and Donation Seeking 101|
|Fundraising - Request to Seek Donations and Fundraise Form||This form is to be completed prior to seeking donations from vendors/individuals as well as before implementing any fundraising activities. This form is located in CI Sync (see instructions above).|
|Fundraising - Report on Fundraising and Donation Efforts Form||
This form is to be completed after donation seeking and fundraising activities have been completed in order to report the outcomes of such efforts. Follow this link to the Report on Fundraising and Donation Efforts Form

|Logo and Artwork Guidelines||
Student organizations should follow these guidelines when creating artwork and logos. Follow this link to the Communication & Marketing resource page and Guidelines for Designing Artwork for Student Organizations
|Meeting Space Request Form||This form is for reserving a meeting space location on campus. This form is located in CI Sync (see instructions above).|
|New Student Organization||This is the process to be completed by student who wish to establish a new student organization. Please follow this link to the process of establishing a New Student Organization.|
|Officer Eligibility Appeals Form||
An officer who is ineligible due to failing to meet the semester GPA requirement may submit an appeal to the Student Organization Officer Appeals Committee for a one-semester (one-time) probationary status to retain the position(s) held at the time the appeal is submitted. Appeals can be accepted no later than the first Thursday of each semester. The Student Organization Officer Appeals Committee will issue decisions within 5 business days after hearing the appeal of each semester. Students may maintain their role as an officer until the appeals committee has issued a final decision. Appeals will not be considered for officers who fall below the CSU mandated requirements or officers of new organizations.
An officer may only appeal if:
The appeal is submitted by the deadline (first Thursday of the semester)
The officer has at least a 2.5 CUMULATIVE grade point average
The officer serviced in the SAME role during the previous semester
The officer has at least a 2.0 semester grade point average (as required by Executive Order 1068)
The officer experienced extenuating life circumstances that lead to him/her receiving below the required 2.5 semester grade point average requirement.

|Poster Request Form||This form is for requesting printing of posters (18" X 24" or 24" X 36"). Poster Request Forms must be received by SLP at least 10 business days prior to the event or activity. Your student organization will be charged $7 for each 18" X 24" poster and $10 for each 24" X 36" poster. This form is located in CI Sync (see instructions above).|
|Posting Approval Form||This form is for requesting to have flyers posted in the Bell Tower for your events/activities. In order to post in other locations on campus (e.g. Broome Library, the Student Union and Housing and Residential Education), student organizations must work directly with each department. This form is located in CI Sync (see instructions above).|
|President Agreement Form||This form must be completed by all student organization presidents by the Friday after the Spring semester ends or no later than 10 days after assuming officer role (in the event of officer changes). When your president's name and email have been updated in the student organization's CISync portal, the new president will be sent the President Agreement. Please follow this link to review the process for adding a president to your organization.|
|Renewal Process 2013-2014||This renewal process must be followed by all student organizations that wish to renew their status for the 2013-2014 academic year. Please follow this link to the 2013-2014 renewal process.|
|Sports Club Insurance Fee Information||
The Sports Club Insurance Fee is a new mandate by the California State University Chancellor's Office. It is now a requirement that sports club members pay a $45 insurance fee each year (for each club that they belong to) as part of the registration process. If students do not pay the fee, they may not join the club. Every club must have at least five members/players who have paid the fee and completed Sports Club Participation Application and the Sports Club Release of Liability and Health Declaration Form in order to be registered with the University.
The Sports Club Insurance Fee covers students who are injured during club practices and games. This means that student are not required to have their own medical insurance in order to be a member of a sports club team (however, it is always recommended so that students have medical insurance for their non sports-club relate illnesses and/or injuries).
|Sports Club Participant Application||This form must be completed by all members/officers annually in order for them to be in a recognized/registered Student Organization. Please note that emergency contact information will be required for completing this form. This form is located in CI Sync (see instructions above).|
|Sports Club Release of Liability and Health Declaration Form||
In addition to the Sports Club Participation Application and Sports Club Insurance Fee, this form must be completed by all members/officers annually in order for them to be in a recognized/registered Student Organization. These documents must be printed, signed and turned into the SEAL Center directly.
|Tabling Request Form||This form is for student organizations that wish to set up a table in the front or back of the Bell Tower for event/activity/organization promotion. This form is located in CI Sync (see instructions above).|
