2019-2020 Forms (Fall 2019 - Summer 2020)

Starting with the 2019-2020 academic year, the Financial Aid & Scholarships office at California State University Channel Islands is introducing Student Forms a new and improved student experience that will make navigating the financial aid process easier.

Providing the best student experience is important to us. We want you to be able to complete your financial aid file on your own time and on the device of your choice.

We are proud to say that if you completed a FAFSA, you are able to:

  • Complete financial aid forms on your phone, tablet or computer
  • Securely upload documents from any device
  • E-sign documents - both you and your parents
  • Manage your financial aid tasks online
  • Receive automated text/email reminders about outstanding tasks and next steps

Step 1: Create Your Account*

This is a one-time account creation. You’ll be asked for some basic information when creating your account. This information is used to validate your identity so that your electronic signature can be used on future forms.

  1. Go to csuci.verifymyfafsa.com
  2. Click ‘Create Account’ link
  3. Enter/confirm your student information matches your FAFSA
  4. Click ‘Create Account’ button

Once your account is created, any required tasks you need to complete will become available.

*CSUCI’s FAFSA School Code 039803 must be listed on your 2019-2020 FAFSA Application.

Step 2: Review Your Tasks and Upload Your Documents

You are now ready to complete your outstanding Financial Aid tasks online and securely upload the required documents from any device with e-sign capabilities. Using your myCI login, go to csuci.verifymyfafsa.com and upload your documents by the May 31st Priority Document Deadline.

  • IRS Tax Transcript Information (PDF, 184KB)
    Students and/or parents who are selected for verification must use the IRS Data Retrieval Tool available through the FAFSA or submit an IRS Tax Return Transcript from the IRS. Please follow the instructions on this sheet to complete this process.
  • FERPA Request to Release Student Information (PDF, 397KB)
    This form must be submitted in person to the Records & Registration office. Use this form to authorize a designated individual to have access to your educational records. Please consult University Policy on the Family Educational Rights and Privacy Act (FERPA) for more information.
  • Payroll Direct Deposit Enrollment Authorization Form (PDF, 57KB)
    Return your completed form to Human Resources, 2801 Lindero Hall. Direct Deposit is a program that allows for the automatic deposit of your net earnings into the financial institution of your choice. All student employees are eligible to enroll. Please consult this Direct Deposit Flyer for more information.
