Forms that must be submitted in person must be turned into Records and Registration, located on the first floor of Sage Hall in the Enrollment Center. 

Forms accepted via e-mail may be submitted to records.registration@csuci.edu. A handwritten signature is required on e-mailed forms, and they must be attached as a legible PDF file (pictures of forms will not be accepted).
