Loan Forms

  • Ineligible PLUS Borrower Form (PDF, 113KB) 
    If a parent is unable to complete a Parent PLUS Loan request form due to reasons such as citizenship status, default status, bankruptcy, incarceration, or family income is limited to public assistance or disability benefits, you may submit this form with supporting documentation for consideration.
  • Parent PLUS Loan Change Form (PDF, 183KB) 
    Submit this form if you wish to make changes to your Federal Direct Loans Parent PLUS Loan.
  • Private Loan Request Form
    If you have applied for a private student loan through your preferred lender, you must also complete this electronic form to authorize the Financial Aid & Scholarships office to certify and process your private student loan.

Satisfactory Academic Progress (SAP) Forms

  • SAP Appeal Form - Summer GPA and Pace (PDF, 167KB) 
    Student request to re-establish financial aid eligibility. This form is designed for students who failed to meet the minimum cumulative grade point average or the acceptable pace rate standard and were placed on financial aid suspension.
  • SAP Appeal Form - Summer Maximum Timeframe (PDF, 217KB) 
    Student request to re-establish financial aid eligibility. This form is designed for students who exceeded their maximum timeframe and were placed on financial aid suspension.

  • IRS Tax Transcript Information (PDF, 184KB)
    Students and/or parents who are selected for verification must use the IRS Data Retrieval Tool available through the FAFSA or submit an IRS Tax Return Transcript from the IRS. Please follow the instructions on this sheet to complete this process.
  • FERPA Consent to Release Student Information
    You have the ability to authorize a designated individual to have access to your education record. Completing the FERPA Online: Authorize to Release form through CI Records allows for instant processing and visibility by University Officials. Please consult University Policy on the Family Educational Rights and Privacy Act (FERPA) for more information.
  • Payroll Direct Deposit Enrollment Authorization Form 
    Return your completed form to Human Resources, 2801 Lindero Hall. Direct Deposit is a program that allows for the automatic deposit of your net earnings into the financial institution of your choice. All student employees are eligible to enroll. Please consult this Direct Deposit Flyer for more information.
