If you have applied for a private student loan through your preferred lender, you must also complete this electronic form to authorize the Financial Aid & Scholarships office to certify and process your private student loan.

Return your completed form to Human Resources, 2801 Lindero Hall. Direct Deposit is a program that allows for the automatic deposit of your net earnings into the financial institution of your choice.
