What is Open Enrollment?

  • Every autumn, the CSU system allows employees to change their medical and dental coverage. Employees may enroll, change plans, or make changes to their own or family coverage.
  • If you are currently enrolled in either the Health Care or Dependent Care Reimbursement accounts (HCRA and DCRA), you must re-enroll every year during the Open Enrollment period.
  • If your current medical and dental coverage meets your needs, and you are not enrolled in HCRA or DCRA you do not need to take any action during the open enrollment period.

When is Open Enrollment?

September 14 - October 23, 2009

What if I miss the deadline?

The deadline for all Open Enrollment benefit forms is 4:00 p.m. on Friday, October 23, 2009. If you miss the deadline, you will need to wait until next fall to change your coverage. If you do not re-enroll in HCRA or DCRA, your coverage will end.

Am I eligible?

You must be employed half time or more in an appointment that exceeds six months and one day. Temporary faculty (AY Classification) must have 6.0 units or more for at least one semester, including the current semester.

Medical Plans

How have medical plans changed for 2010?

Health Maintenance Organizations (HMOs)

Blue Shield

  • No changes.

Blue Shield NetValue

  • Now also available to residents of Ventura, Los Angeles, Orange, and San Luis Obispo counties, and parts of Riverside, San Bernardino, Kern, Fresno, Madera, and Santa Barbara counties.

Kaiser Permanente

  • Eliminating chiropractic benefits.
  • Increasing co-payment for 31-100 day supply of prescription drugs.

Preferred Provider Organizations (PPOs) - Administered by Anthem Blue Cross

PERSCare, PERS Choice and PERS Select

  • Expanding benefits for transplants.

Peace Officers Research Association of California (PORAC) (only for Unit 8 employees who are dues-paying members of the Association)

  • Adding coverage for HIV testing.
  • Expanding benefits for transplants.

How do I make changes to my Medical Plan?

  • Click here to access the CSUCI Benefit Worksheet.
    • Print and complete this form.
    • Make sure to have Social Security Numbers for your spouse or domestic partner if you are adding them to your plan.
    • If you are switching medical plans, please make sure to list any and all dependents whom you would like covered, whether or not they are included in your current plan.
  • Submit a copy of your Marriage Certificate if adding your spouse; complete and submit a copy of the approved Declaration of Domestic Partnership if adding a domestic partner.
  • Submit a copy of a Birth Certificate if adding a dependent child (under 23 years of age).
  • Print and complete the Declaration of Health Coverage (HBD 12-A) Form.

Dental Plans

The CSU offers two types of dental insurance plans, which are employer-paid: Delta Dental Premier/PPO and DeltaCare USA HMO.

What are the Dental Plan changes for 2010?

    • There are no changes to the plan.
    • Delta Dental (PPO) and DeltaCare USA continue to be employer-paid.

FlexCash

Can I be compensated if I waive CSUCI benefit coverage?

  • All employees eligible for medical and dental insurance may waive their CSU coverage and receive cash if they are covered under other non-CSU medical and/or dental insurance. (Proof of coverage is required.)
  • If you are covered as a dependent of another CSU enrolled employee you are not eligible to participate in this program. If you are, however, a covered dependent under your retired spouse/parent’s CalPERS health plan, then you are eligible for the FlexCash plan.
  • If you elect to receive cash in lieu of medical and/or dental coverage(s), the following monthly cash payment(s) will reflect in your pay warrant:
    • Waive both medical and dental -- $140 per month
    • OR waive only medical -- $128 per month
    • OR waive only dental -- $12 per month
    • A $2.00 after-tax administrative fee will be deducted from your pay each month you participate

How do I enroll in FlexCash?

  • Complete sections 1-6 and print the FlexCash Enrollment Form.
  • Sign and date the form.
  • Copy your insurance card, showing proof of insurance, and include the copy with the completed form.

Dependent or Health Care Reimbursement Accounts

Are you missing an opportunity to put more money in your pocket?

  • These programs allow you to pay for dependent care and/or health care out-of-pocket expenses with pre-tax dollars. Contributions to this plan are deducted from your pay prior to federal, state, and Social Security (FICA) taxes being withheld.
    • Health Care Reimbursement Accounts (HCRAs) are designed to assist you in paying deductibles or co-payments with pre-tax dollars. Click here for information about planning for your HCRA.
    • Dependent Care Reimbursement Accounts (DCRAs) are designed to assist you in paying for child care or adult day care for dependent adults, also with pre-tax dollars. Click here for information about planning for your DCRA.
  • You must estimate your eligible expenses very carefully. Any funds left in your HCRA and/or DCRA after your expenses have been paid for the year will be forfeited.
  • You may enroll for the first time during Open Enrollment.
  • You must re-enroll in these programs each year.
  • Administrative fee remains $1.00 per month for each plan.

CSU Health Care Reimbursement Account (HCRA)

Eligible Expenses:

Medical, dental, and vision expenses eligible to be reimbursed by the HCRA Plan include uninsured and medically necessary expenses incurred by you and your dependents. Expenses must be incurred for the diagnosis, cure, treatment, or prevention of disease; for treatment affecting any part or function of the body; or to alleviate or prevent a physical defect or illness. Expenses incurred solely for cosmetic reasons or expenses that are merely beneficial to a person’s general health are not eligible for reimbursement. CSU Dependent Care Reimbusement Account (DCRA)

Eligible Expenses:

Dependent care expenses will be eligible for reimbursement through your DCRA provided the care is required in order for you to be gainfully employed. If you are married, your spouse must also be employed unless he or she is disabled or a full-time student. Eligible Dependents:

Eligible dependents for whom DCRA reimbursements can be claimed are: (a) a child under the age of 13, for whom you or your spouse can claim dependent status on your income tax return, (b) a financially dependent member of your household who is physically or mentally disabled and who regularly spends at least 8 hours each day in your home, and/or (c) a spouse who is physically or mentally unable to care for him/herself.

HCRA and DCRA Eligibility

Represented and non-represented employees (regardless of time base or duration of appointment) are eligible to participate in the HCRA/DCRA Plans with the exception of student assistants, graduate assistants, summer session faculty, rehired annuitants including FERP participants, and intermittent/hourly staff.

How do I enroll/re-enroll in DCRA/HCRA?

  • New and continuing participants must complete the Open Enrollment form.
  • Print the HCRA/DCRA Enrollment Authorization Form and complete sections 1 - 5 and section 7.
  • Sign and date the form.
  • All forms are due by 4:00 p.m. on Friday, October 23, 2009. Note: Due to the confidential nature of this information, we recommend bring your enrollment forms and copies of necessary documents to Human Resources Programs, (Solano Hall, room 1123).
  • Enrollment becomes effective January 1, 2010.
