Instructions for Faculty, Staff, and Student Workers

  1. The form itself must be routed and signed using the CPAL Form in Adobe Sign. It is recommended that you review the form and all of these instructions before routing it via Adobe Sign.

  2. Only Faculty and Staff may initiate the routing of the CPAL Form in Adobe Sign. Student Workers may ask their supervisor to initiate the routing of the Non-Faculty CPAL form on their behalf.

  3. To begin the routing process, choose the appropriate button below for your role:

    1. Faculty CPAL Form in Adobe Sign

    2. Non-Faculty (Staff) CPAL Form in Adobe Sign

  4. If the Faculty or Staff is not already logged into their Adobe Sign account, they will be prompted to do so. This login process prompts you to enter your CSUCI email address. This is not to be confused with the email routing described in step 6.

  5. Enter your CSUCI email address (e.g., peter.mosinskis@csuci.edu) and click the "Sign In" button. If prompted, choose "Company/School Account"

  6. You will be prompted to login via myCI. Enter your Dolphin Name and Dolphin Password when prompted. You will also likely need to complete your Duo two-factor authentication.

  7. Review the routing instructions, which vary slightly for faculty and staff:

    1. Faculty will need to enter the following email addresses:

      1. Faculty Member: email address for the requesting Faculty member (put your own email address if you are the one making the request)
      2. Department Chair: email address of the requesting faculty member's department chair
      3. Dean: email address of the dean for the requesting faculty member's department
      4. Academic Personnel Designee: enter donna.flores@csuci.edu
    2. Non-faculty (staff) will need to enter the following email addresses:

      1. Employee Requester: email address for the requesting staff member or student worker (put your own email address if you are the one making the request)
      2. Appropriate Manager: email address of the requesting employee's manager
      3. HR Designee: enter christine.girardot@csuci.edu
  8. Click the "Send" button

  9. If you are the first person to receive the agreement (the employee requester), you will be prompted by Adobe Sign to fill it out. You will also receive an invitation via email to complete the agreement.

  10. Fill out all required fields (they are marked with an *). Faculty members are able to attach a document in the "File attachment field" to include a copy of their schedule.

  11. Click the "Click to Sign" button at the bottom of the page. Choose to sign the agreement by typing your name, drawing your name, or one of the other available options.

  12. You will see a success message page once the agreement has been signed. Once signed, the form will be automatically routed via email to the next person in the list for signature. On the success message page, you can click the "Manage this agreement" link to view the status of its signing by others.

  13. Once the form has been completed by all signers, all signers of the agreement will receive an email with a link to view and download a copy of the completed signed agreement.
