Legal Name Change and Name Corrections

If your name has changed, you will need to fill out the Legal Name Change/Name Correction Request (PDF, 159KB)

Correction of Social Security Number or Date of Birth

If you need to correct your social security number or date of birth you will need to fill out a Correction of Demographic Information on Student Record (PDF, 206KB)

Documentation is required for both forms. Please see form for more information on required documentation. Submit completed form to the Enrollment Center, Sage Hall 1020 with all required documents attached. The form must be mailed or brought in person only. No email or faxes allowed. If mailing, you must attach a copy of photo ID.

PLEASE NOTE:If you currently or have ever been employed at CSU Channel Islands (faculty, staff, student assistant or intern), your name change must be completed with Human Resources. They will process your name change campus-wide.

Change of Address/Change of E-mail Address

If you have recently moved, or are changing your mailing address, you will need to log in to myCI and visit the Student Center in CI Records to change your address on-line by following the How to Update Your Personal Contact Information (PDF, 429KB) guide. If you are not a current student, you may submit a Change of Address form (PDF, 71.0KB) to update your student records at the Enrollment Center. It is important for you to have the correct address on file as the campus mails information regarding registration, financial aid, graduation and other important notifications.

PLEASE NOTE:If you currently or have ever been employed at CSU Channel Islands (faculty, staff, student assistant or intern), your must complete a change of address with Human Resources (to receive W2 and other employment information) AND online through myCI at the Student Center in CI Records (to receive student related information including your diploma).

What happens next?

The updates to your myCI log in, as well as CI Learn (Blackboard), dolphin email, etc. will be updated every two weeks. If  you are changing/correcting your name, you will be notified by email and provided with your new log in. The only exception is during grade processing. Name changes will processed after grades are assigned. Correction of demographic information forms and Change of Address forms will be processed weekly.
